The SINC Social Worker position offers an exciting opportunity for a social worker with extensive experience or a new graduate looking to start their careers.
We are currently looking to recruit a Social Worker to join our team in the Seniors in Networking Community Program to deliver Commonwealth Home Support Funded Allied Health Services to seniors living in their own homes.
This service delivers social work services to older people throughout areas of NSW, working both with individuals and with groups to assist older people to regain or maintain their independence. The role offers a variety of work environments, with interventions taking place in individual’s homes and in the wider community. The key responsibilities for the SINC Social Worker will be to work with older people who have been referred by My Aged Care to the SINC Program for Allied Health Services. Our SINC Social Workers focus on, person centred, wellness and reablement and works within a restorative care framework providing individualised support and interventions to assist people with being able to remain in their homes and as part of their community. if you would like to work with us to make a positive difference in people’s lives - we want to hear from you!!!
