CHOICES & Aged Care Social Activities

CatholicCare’s CHOICES & Aged Care Services facilitate a range of social activities, with a focus on both social engagement and skill development.

Activities are based primarily out of Tenison Woods House (TWH), which is located at 57 MacArthur Ave in O’Connor.

Activities are open to individuals who:

• Have an NDIS Plan, with available funds in their core or capacity building budgets
• Are part of the Commonwealth Home Support Programme (CHSP),with a referral code for Social Support Individual and/or Social Support Group
• Have a Home Care Package, with available funds

Fees to attend the group will vary depending on your funding type and the duration of the group.

Certain activities may attract additional costs depending on the type of activity, these costs are to be paid by the individual on the day and are listed next to each activity.

• Phone 6162 6100 – please ask to speak with the CHOICES & Aged Care Team and leave a message if we are unable to answer
• Email ChoiceandAgedCareServices@catholiccare.cg.org.au leaving your name and best contact number, so we can get back to you

Referral

A referral can be made by an individual or on an individuals behalf by a friend or family member, another health professional (such as their GP) or another service provider.

Referrals can be made by completing the referral form or contacting our team on 6163 7600.
